Your membership ID number. (Disregard any zeros that may appear at the beginning of your membership ID number.)

Your last name is your password. Be sure to capitalize the first letter of your last name as the field is case sensitive. If you have a space in your last name such as Mac Donald, eliminate the space and type MacDonald
